Origin
The idea stems from boswellic acids' ability to inhibit 5-lipoxygenase and from Ayurvedic use of the resin for joint problems.
What Science Says
A 2020 meta-analysis of 7 RCTs (545 osteoarthritis patients) found Boswellia reduced pain (VAS -8.3 points) and stiffness and improved function. However, a 2024 meta-analysis of 13 studies found no significant overall effect on WOMAC or pain scores because of high heterogeneity, with benefit emerging only in placebo-controlled subgroups, and a 2026 network meta-analysis found improvements mostly with modified formulations in mild-to-moderate knee OA. Evidence supports a modest symptomatic benefit in knee osteoarthritis, but not a proven NSAID-equivalent replacement.
